Dental amalgam, "silver fillings", has been around for over 100 years. It has been used to "fill" teeth no matter how big the area of decay was or how ugly it made the tooth look. Another problem with the silver amalgam is it requires removal of more tooth structure past the decay to create "undercuts" to lock the filling in the tooth. These undercuts may further weaken the tooth and make it more likely to fracture.
Today, with newer materials such as dental composites and porcelain, we can place restorations that not only replace the decayed area of the tooth, but add strength back to the tooth by bonding to the remaining tooth structure. Bonding reduces the amount of tooth that needs to be removed, therefore conserving natural tooth structure for the strength. Dental composites are indicated for smaller fillings and can be placed in one appointment.
Porcelain is used for layer restorations that replace large areas of the tooth, but not so much that the tooth requires a crown. Porcelain inlays and onlays are fabricated in a dental lab for maximum strength, ideal contours and esthetics. Porcelain inlays and onlays require two visits to complete.
